Application Form for Experienced Presenters
Required Submissions:
- Completed SDE Presenter Application & Information Form (Download this form. It is a Microsoft Word format file)
- Professional resume
- A two-three paragraph presenter bio (sample)
- Submit a minimum of six 75-minute workshop descriptions that you present (sample). Each workshop description should:
- Include a clear title
- Specify the grade levels on which you can present
- Be 100-200 words
- Specify the length of time for which you could present:
75 minutes, 3 hours, 6 hours, Other
Recommend Submissions
(not required but will greatly expedite the review process):
- Testimonials (from past workshop participants)
- Video/DVD of presentation to educators
- List of authored publications, materials, CD's etc…
- List of recent presentations
- List of upcoming presentations
